TENANT’S CONSENT: By submitting this request, I/we request the above maintenance attended to and I/we hereby authorize the Landlord to enter my/our suite in my/our absence. Should it be necessary to use a service agency other than the Landlord or should more than one visit be required for correction, I/we also authorize the Landlord and/or the service agency to enter my/our suite.
